Assist the investigator by recording in a standardized format the specific set of data necessary to support an investigation and subsequently report it to the state.
Assure each step of an investigation has been completed prior to closing the investigation. This documentation tool may be placed in the investigation folder. Remember the adage: “not documented, not done.”
Provide written information about the disease being investigated. These fact sheets may be sent to any party associated with an investigation and contain information specific to the disease and methods used to control it.
Assist in requests for information and/or notifications of individuals or groups about possible exposure to a disease.
Offer a visual guide for the investigator showing each step of an investigation.
